New Hampshire Building Capacity for Transformation Medicaid Waiver, Special Terms & Conditions

On January 5, 2016, the federal Centers for Medicare & Medicaid Services (CMS) approved New Hampshire's Medicaid waiver application for a project intended to develop a sustainable integrated behavioral and physical health care delivery system. The demonstration period runs through December 31, 2020. Through the demonstration, “Building Capacity for Transformation,” the state plans to partner with regionally-based Integrated Delivery Networks (IDNs ) that are coalitions of behavioral health and other health care and community provider organizations working collaboratively.
