Ohio Medicaid Sample Episode Of Care Provider Report

This document released in January 2016 the Ohio Office of Health Transformation and the Medicaid program is a sample of the reports to be issued to primary care patient-centered medical homes (PCMH). The state is in the process of implementing episode of care bundles. During 2016, 20 episodes were defined and launched across payers, including behavioral health.
