Four Oaks Family and ChildrenÂ’s Services submitted the only proposal to the Iowa Department of Human Services RFP seeking a System of Care for Linn and Cerro Gordo Counties (MHDS 12-013). The goal is to develop a set of core services for children up to age 21 with serious emotional disturbance, replicating a System of Care operating in Northeast Iowa. These core services are to include intensive care coordination, family peer support, crisis services, intensive community-based treatment, and the use of the health home model of service delivery.
