Four Oaks Family and Children’s Services Proposal to Iowa MHDS 12-013 For System Of Care In Linn and Cerro Gordo Counties, Iowa

Four Oaks Family and Children’s Services submitted the only proposal to the Iowa Department of Human Services RFP seeking a System of Care for Linn and Cerro Gordo Counties (MHDS 12-013). The goal is to develop a set of core services for children up to age 21 with serious emotional disturbance, replicating a System of Care operating in Northeast Iowa. These core services are to include intensive care coordination, family peer support, crisis services, intensive community-based treatment, and the use of the health home model of service delivery.
