Be Well Partners in Health

In February 2013, Be Well Partners in Health submitted a winning proposal to become a Care Coordination Entity (CCE). Lead partners include MADO Management LP, Bethany Homes and Methodist Hospital, Norwegian American Hospital and Neumann Family Services. The target population includes adults with severe mental illness (SMI) and chronic health conditions, including substance abuse, on the North Side of Chicago. The model will test care coordination organized by a nursing facility group, with its unique insight into long-term services and an additional focus on care coordination services within long-term care settings.
