National List: Nursing Homes Targeted for High-Risk Pressure Ulcer and/or Physical Restraint Improvement

January 2008 This resource is a 94-page list of nursing homes identified by the federal Centers for Medicare and Medicaid Services (CMS) as not meeting CMS benchmarks for making improvements in their rates of pressure ulcers and use of physical restraints. The list was compiled as a reference for a solicitation for CMS Quality Improvement Organizations (QIOs).
