Colorado Choice Transitions Consumer Fact Sheet

In early 2013, the Colorado Department of Health Care Policy and Financing (HCPF) is launching its Money Follows the Person (MFP) demonstration program, Colorado Choice Transitions (CCT). The program will help Medicaid beneficiaries living in long-term care facilities move from those facilities to a community placement supported by enhanced home- and community-based services (HCBS). HCPF anticipates helping 500 people transition to the community by March 31, 2016. This fact sheet explains the program.
