In March 2013, the Agency for Healthcare Research and Quality (AHRQ), along with professionals at Boston University, prepared a toolkit for the U.S. Department of Health and Human Services that aims to reduce hospital readmissions and post-hospital emergency department visits. This set of activities and materials was developed to help hospitals, specifically those with diverse populations. The report explains why hospitals would want to re-engineer their discharge processes, provides evidence of the RED's impact, and discusses each tool.
