January 9, 2008 November 13 through 16, 2006, the United States Department of Justice conducted an on-site review of care and treatment at both Oregon State Hospital (OSH) campuses. The team of reviewers found that conditions and practices at OSH violate its residents' rights. In particular, the following areas were problems: OSH failed to protect patients from harm Failure to protect patients from harm Failure to provide appropriate psychiatric and psychological care and treatment Failure to use seclusion and restraints in a manner consistent with generally accepted professional standards Failure to provide adequate nursing care Failure to provide discharge . . .
