The Centers for Medicare and Medicaid Services (CMS) is launching a new effort to reduce hospital readmission rates for long-stay nursing facility residents by linking care coordination programs with nursing homes. CMS has set aside $128 million for the new four-year initiative, which will distribute a total of seven grants to organizations that apply to become "enhanced care and coordination providers." CMS released the request for applications on March 15, 2012, with the applications due by June 14. The projects are slated to begin August 25, 2012 and will run through August 24, 2016.
