Illinois DCFS Care Coordination Pilot Presentation
In this presentation given March 20, 2014, the Illinois Department of Children and Family Services (DCFS) outlined a Care Coordination Pilot Program taking place in Champaign, Ford, Iroquois, and Vermillion counties. In these communities, the goal is to develop additional home- and community-based services (HCBS) for DCFS wards to ensure that wards are stabilized in the least restrictive setting possible. The three-year pilot is taking place in conjunction with Healthcare and Family Services (HFS) and the Department of Human Services (DHS).
