On April 8, 2021, the Center for Medicare & Medicaid Services (CMS) Center for Medicare & Medicaid Innovation (Innovation Center) published this notice of direct contracting entities participating in performance year 2021 (PY2021) of the Global and Professional Direct Contracting (GPDC) model.
