In this guidance released on April 11, 2016, the federal Centers for Medicare & Medicaid Services (CMS) explained details about phase two of the Initiative To Reduce Avoidable Hospitalizations Among Nursing Facility Residents. In phase two, the participating organizations will implement new payment models intended to help reduce the prevalence of hospitalization among nursing facility residents.
