The Medicare Independence at Home Demonstration encourages primary care practices to provide home-based care for up to 10,000 fee-for-service (FFS) Medicare beneficiaries with multiple chronic conditions. Eligible FFS beneficiaries must have two or more chronic health conditions, a non-voluntary hospital admission in the past year, received acute or sub-acute rehabilitation services in the past year, and have on-going needs for help with activities of daily living. The 16 organizations will test whether home-delivered primary care services and assessments increase the quality of care and reduce costs for eligible participants . . .
