In June 2012, Macon County Mental Health Board (lead partner) submitted a winning proposal to become a Care Coordination Entity (CCE). The target population includes adults with serious mental illness, seniors with chronic illness, including people eligible for both Medicare and Medicaid, and children and family members of adult Medicaid enrollees in Macon County, which is in southern Illinois. The proposed model tests care coordination organized by a county-based mental health organization, and will be used to demonstrate the effectiveness of care coordination led by mental health providers.
