In June 2013, Healthcare Consortium of Illinois submitted a winning proposal to become a Care Coordination Entity (CCE). The lead partners is the Healthcare Consortium of Illinois, a community-based, non-profit organization. The target population includes seniors and their eligible family members in 13 zip codes on the South Side of Chicago. The model will test care coordination organized by a community-based organization that promotes the concept of "networks within networks" with its base of hospitals, physicians and social service organizations.
