The Path to Transformation: Illinois 1115 Waiver Proposal

On June 4, 2014, the Illinois Department of Healthcare and Family Services (HFS) submitted a Medicaid 1115 demonstration waiver proposal to the federal Centers for Medicare and Medicaid Services (CMS) seeking approval to consolidate nine long-term services and supports (LTSS) waivers, as well as other global changes.
