Maryland Total Cost Of Care Model: Primary Care Program Request For Applications

The request for applications was released on June 8, 2018, by the federal Center for Medicare and Medicaid Innovation (CMMI) for the Maryland Department of Health's all-payer Total Cost of Care initiative, called the Maryland Primary Care Program (MDPCP). Applications were due by July 23, 2018. CMMI will select care transformation organizations (CTOs) to work with primary care provider organizations participating in the MDPCP.
