CMS Termination Letter To Arizona Department Of Health Services Regarding State Hospital Deficiencies

In this letter dated June 17, 2015, the federal Centers for Medicare & Medicaid Services (CMS) notified the Arizona Department of Health Services (ADHS) that the Arizona State Hospital had failed a complaint verification inspection and was out of compliance with Medicare conditions of participation. CMS informed ADHS that the hospital would lose its Medicare certification unless ADHS submitted an acceptable plan of correction.
