Centennial Care: Care Coordination Frequently Asked Questions

In July 2013, the New Mexico Human Services Department released a care coordination fact sheet for the Centennial Care Medicaid managed care program that launches January 1, 2014. The fact sheet defines Centennial Care, lists the managed care organizations selected to provide services, timetable for launch, eligibility, benefits covered, and defines community benefits. Care coordinators are employed by and assigned by the Centennial Care managed care organizations.
