As of February 16, 2012, the federal Department of Health and Human Services Center for Consumer Information and Insurance Oversight (CCIIO) said that 44 states, the District of Columbia, and three U.S. territories had effective rate review programs for at least one insurance market. This fact sheet describes the Rate Review program and state progress implementing state-level Rate Review programs.  The Patient Protection and Affordable Care Act of 2010 (PPACA), prohibits individual and small group market health insurance plans from raising premium rates more than 10% without approval from a state or federal Rate Review program . . .
