Arizona Department Of Health Services Plan Of Correction For Arizona State Hospital

This document dated July 14, 2015, is a plan of correction submitted by the Arizona Department of Health Services (ADHS) to the federal Centers for Medicare & Medicaid Services (CMS) to address serious deficiencies in operations of the Arizona State Hospital identified by CMS in a May 2015 complaint verification inspection. CMS approved the plan of correction on July 20, 2015.
