On July 11, 2013, the U.S. Department of Health and Human Services published a contract that specifies contract management and readiness review requirements for a dual eligible demonstration capitated model contract. Contract covers enrollment activities; covered services; care delivery model; comprehensive assessments and individualized care plan; ICT (Interdisciplinary Care Team) discharge planning participation; provider network; network management; enrollee access to services; CMS and Massachusetts EOHHS responsibilities; payment and financial provisions; and additional terms and conditions.
