August 2007 The August 2007 Connecticut Medicaid disease management request for proposals (DSS081508_DM_RFP) contained several errors, according to the Disease Management Purchasing Consortium International (DMPC). The alleged errors include reported state payments for cardiac disease in which for 1,314 people with congestive heart failure in the Medicaid fee-for-service program in 2005, the state paid $793,970, or about $604 per person.
