August 2007 The August 2007 Connecticut Medicaid disease management request for proposals (DSS081508_DM_RFP) contained several errors, according to the Disease Management Purchasing Consortium International (DMPC). The alleged errors include reported state payments for cardiac disease in which for 1,314 people with congestive heart failure in the Medicaid fee-for-service program in 2005, the state paid $793,970, or about $604 per person. Further, according to the RFP, the state paid $2.2 million for care provided to 904 people following a heart attack, about $2,456 per person.
