A New York City Department of Investigation Examination of 11 Child Fatalities & One Near Fatality

August 9, 2007 The report was conducted by the New York City Department of Investigation (DOI) and the Administration for Children's Services (ACS) at the request of New York Mayor Michael Bloomberg. The DOI reviewed ACS actions in the cases of 11 children who died and one who nearly died between October 2005 and July 2006 after claims of neglect or abuse had been reported to ACS.
