Missouri Medicaid and the Department of Mental Health launched two types of health homes in January 2012; community mental health centers (CMHCs) provide Healthcare Homes for individuals with mental illness and Primary Care Chronic Conditions Healthcare Homes serve individuals with other chronic health conditions. The report documents the CMHC Healthcare Home progress between January and June 2012 to implement the model; it also reports the number of consumers enrolled and quality metrics for the period.
