On October 25, 2012, the State of Washington released a memorandum on establishing a Federal-State partnership between the Centers for Medicare & Medicaid Services (CMS) and the State of Washington for a health home demonstration. The memorandum includes information on the purpose of the Memorandum of Understanding (MOU); program design/operational plan; definitions; details of the state demonstration area; Medicaid authorities and waivers; performance payments to the state; and demonstration parameters.
