DRAFT: Design of the CMS Medical Home Demonstration

October 3, 2008 The report specifies the design of the upcoming Medicare Medical Home Demonstration. The Centers for Medicare and Medicaid Services (CMS) will announce the eight states selected to participate in December 2008. Physician practices in those states are encouraged to apply to participate. The demonstration will start in 2010 and end in 2012. Physicians will receive payments for providing care management services for Medicare beneficiaries enrolled in the demonstration.
