List Of Medicare Primary Care First Model Participants

The list was released on November 19, 2020, by the Centers for Medicare & Medicaid Services. It includes the names of 916 primary care practices and 37 regional health plan partners to participate in the Primary Care First model beginning January 1, 2021. The program will be offered in 27 states. Primary Care First Model Option is a set of voluntary five-year payment options that reward value and quality by offering an innovative payment structure to support delivery of advanced primary care.
