Iowa DHS RFP For Linn and Cerro Gordo Counties System of Care MHDS 12-013

December 13, 2011 On March 17, 2011, Iowa Department of Human Services (DHS) released a request for proposals for contractors to provide System of Care services in Linn and Cerro Gordo Counties. The System of Care's goal is to develop a set of core services that will support the return to Iowa of children with serious emotional disturbance who are currently placed in out of state facilities. These core services are to include intensive care coordination, family peer support, crisis services, intensive community-based treatment, and the use of the health home model of service delivery.
