Introduction To The Ohio Episode-Based Payment Model

In this document released in December 2015, the Ohio Office of Health Transformation presented an overview of its cross-payer plan to implement episode-based bundles of care for specific conditions treated in primary care patient-centered medical homes (PCMHs).
