On February 24, 2014, the Health Home Information Resource Center released a report on the Affordable Care Act (ACA) creating the Medicaid health home state plan option to coordinate primary and acute physical and behavioral health care and long-term services and supports (LTSS) for eligible Medicaid beneficiaries.
