Illinois State Plan Amendment Number 11-007: Implement The Integrated Care Program

Approved on February 14, 2012, this state plan amendment allows Illinois’s Medicaid program to implement the Integrated Care Program in Cook County as well as the surrounding border counties. The Integrated Care Program is a voluntary program that provides managed long-term services and supports to the elderly and physically disabled populations.
