Iowa Health & Wellness Plan Healthy Behaviors Program Toolkit For Providers

In April 2015, the Iowa Medicaid Enterprise released this toolkit for providers explaining the healthy behavior incentive for members of the Iowa Medicaid Health and Wellness Plan. The Iowa Health and Wellness plan is an alternative Medicaid expansion plan for childless adults, ages 19-64 with income below 138% of the FPL.
