Massachusetts State Innovation Model

This document reflects the grant proposal submitted by the Commonwealth of Massachusetts to the Centers for Medicare and Medicaid Innovation on February 5, 2013. Massachusetts proposes to use grant funds to accelerate the migration to a statewide multi-payer model in which providers, particularly primary care providers, assume accountability for the quality and cost of care provided to their patients across the delivery system.
