Ohio Application For State Innovation Model Test Assistance

As a result of the State Innovation Model (SIM) design process, Ohio achieved multi-payer agreement across Medicaid, state employee, and commercial health plans to launch episode-based payments statewide in November 2014, and to adapt Southwest Ohio’s Comprehensive Primary Care Initiative (CPCI) for a statewide roll-out of patient-centered medical homes (PCMH) beginning in 2015.
