California’s San Diego County Health and Human Services Agency, Office of Homeless Solutions awarded a contract to PATH (People Assisting The Homeless) to provide up to 12 months of comprehensive care coordination, service navigation, peer support, housing, and housing-related assistance to Persons Experiencing Homelessness, or at risk of homelessness, who are justice-involved (i.e., incarcerated or recently released from custody), and are living with any of the following: mental illness, chronic health conditions, intellectual or developmental disability, Traumatic Brain Injury, Substance Use Disorder, physical limitations, organic brain illness (such as dementia), and/or are considered medically complex . . .
