Released on September 25, 2013, this letter was written to inform that Hawaii’s request for a five-year renewal of the operating demonstration, entitled QUEST Expanded, (Project Number 11-W-00001/9) was approved by the Centers for Medicare & Medicaid Services (CMS) in accordance with section 1115(a) of the Social Security Act. Beginning October 1, 2013, this demonstration was entitled QUEST Integration. The accompanying special terms and conditions (STCs), waivers and expenditure authorities were effective October 1, 2013 through December 31, 2018 and superseded the STCs, waivers, and expenditure authorities on October 1, 2013 . . .
