Readmissions For Medicaid Patients: State-Level Benchmarks And Initiatives

In September 2012, the Medicaid Medical Directors Learning Network made a presentation at the 2012 AHRQ Annual Conference on the Medicaid readmission data project. The presentation covered specifications for reporting and analysis; results, and state readmission policies. The analysis was based on state-level summary data for 18 states.
