On November 22, 2013, the Centers for Medicare & Medicaid Services (CMS) Denver Regional Office completed its review of South Dakota's State Plan Amendment (SPA), and this is the report detailing the process. It includes and describes changes to the South Dakota Medicaid State Plan concerning the implementation of Health Homes for Medicaid recipients with complex health care needs.
