Dylan Holm brings over five years of data analytics and informatics experience to the OPEN MINDS team.

Mr. Holm was recently the Manager of Health Informatics at Integrated Healthcare Partners (IHP), a healthcare staffing company in South Kingston, Rhode Island. In this role he worked with eight disparate organizations to coordinate data reporting, quality improvement projects and communicate state guidelines for a large Medicaid Accountable Care Organization (ACO).

He assisted in the development and execution of health system transformation projects, quality improvement initiatives, including completion of reporting requirements for IHP to secure $7 million in funding. Mr. Holm led committees to coordinate activities for both quality and data analytics and clinical leadership.

Before Integrated Health Partners, Mr. Holm was the Quality Reporting Analyst for Blackstone Valley Community Health Center in Pawtucket, Rhode Island. Here he led a committee to improve clinical performance and presided over projects which resulted in a 10% increase in the percentage of children who received a full course of vaccines.

Mr. Holm wrote PDSA (Plan-Do-Study-Act) cycles to guide change in clinical procedures, with input from clinicians and other staff. He also affirmed the Center’s Patient Centered Medical Home designation with the American Association of Family Physicians by reporting relevant data and engaging in performance improvement activities.

Mr. Holm has a Bachelor of Science in Actuarial Mathematics from Bryant University.
