Minnesota Seeks Mobile Person-Centered Teams For Behavioral Health-Related Hospital Transition Services

The Minnesota Department of Human Services (DHS) released a request for proposals (RFP) for mobile transition teams to help coordinate community-based aftercare for people being discharged from a psychiatric hospitalization or an emergency department hold. The mobile person-centered teams will facilitate effective transition of adults and children who no longer need a hospital level of care and assist them in transitioning to a lower level of care, including psychiatric residential treatment facilities (PRTFs), group homes, or foster homes.
