Intermountain Health & Story Health Partner Implement Between-Visit Care Management For Heart Failure Patients

On January 19, 2023, a seven-state health system, Intermountain Healthcare, and Story Health announced a strategic partnership to improve access to specialty care management for Intermountain’s patients with heart failure. Intermountain Health will use Story Health’s digital specialty care platform and health coaching to help scale personalized patient care pathways. The goal is to improve patient adherence to their treatment plans. The partners intend to publish early outcomes in late 2023.
