UC San Diego Health & 211 San Diego Pilot Post-Discharge Medical Transportation Referrals To Improve Receipt Of Follow-Up Care

A pilot project in San Diego, California intends to help consumers with transportation barriers that hinder access to medical care. The pilot is a collaboration between UC San Diego Health (an academic medical center) and 211 San Diego, the county’s region-wide, non-profit referral point for a variety of community, health, social and disaster services. Both entities are participating in the San Diego Community Information Exchange (CIE).
