AmeriHealth Caritas DC Partners With Ounce On SDOH Initiative For Medicaid Beneficiaries, To Include Care Coordination & Wraparound Services

AmeriHealth Caritas District of Columbia (DC), a Medicaid managed care organization (MCO) serving the District, is partnering with Ounce, a company bridging the gap between health and housing, on an initiative to identify and address its Medicaid members’ needs for social determinants of health (SDOH). The partnership started in November 2022 as a three-year contract with renewal options. Ounce’s trained community health workers (CHWs) provide a range of care coordination and wraparound services to AmeriHealth Caritas DC members.
