Minnesota Releases $8 Million ‘Live Well At Home’ Procurement To Fund HCBS Innovation
On February 12, 2024, the Minnesota Department of Human Services Aging and Disability Services Division (State) released a request for proposals (RFP) to award $8 million in grants for the Live Well at Home program, which provides services to help older adults remain in the community. The state uses this program to support innovation and new approaches in housing and home and community-based services (HCBS) development. The grants provide one-time, start-up funds. They can also be used to develop and support core HCBS provider organizations. Proposals are due by April 5, 2024.
