Kaiser Permanente & Town Hall Ventures Launch Habitat Health To Offer California PACE Services In 2025

On March 27, 2024, Kaiser Permanente and Town Hall Ventures launched Habitat Health, which intends to offer a Program of All-inclusive Care for the Elderly (PACE) in Los Angeles and Sacramento, California in 2025. Habitat Health plans to expand nationally. Habitat Health is supported by Kaiser Permanente, Town Hall Ventures, and New Enterprise Associates.

PACE is a community-based model of care that launched in the 1970s for low-income Medicare and Medicaid beneficiaries aged 55 years and older with disabilities who live in the community. It provides enrollees with fully integrated health, social care, and long-term care . . .
