Humana Expands VBC Kidney Care Agreement With Interwell Health To Florida

Humana expanded its 13-state value-based care (VBC) arrangement with Interwell Health for kidney care to include Florida. Humana Medicare Advantage HMO and PPO members in Florida living with chronic kidney disease (CKD) and end-stage kidney disease (ESKD) will be able to access kidney care management from Interwell Health. Humana’s VBC model aligns financial reimbursement with positive health outcomes.
