Sentara Health Expands Care Coaching Solution To Its Commercial & Marketplace Members With Physical & Behavioral Health Needs

Sentara Health Plans is expanding its use of a coaching solution for its commercial, self-funded, and Marketplace members in Virginia to help them address physical and behavioral health challenges and social needs. The solution is the Ontrak Engage platform; it will be offered to eligible members on an opt-in basis. By offering the coaching, Sentara Health anticipates closing gaps in care and improving plan performance on the standardized Healthcare Effectiveness Data and Information Set (HEDIS) performance measures that health plans use to gauge the quality of care they provide.
