Habitat Health With Partner Kaiser Permanente Opens Its First PACE Center In Sacramento
On January 2, 2025, Habitat Health with its partner Kaiser Permanente opened its first Program of All-Inclusive Care for the Elderly (PACE) center in Sacramento, California. The PACE center in Sacramento is the first of several centers that Habitat Health and Kaiser Permanente plan to open together in California, subject to applicable regulatory approvals. Enrollment started in mid-December 2024, and the center officially opened in January 2025. There is no cap on enrollment.
PACE serves community-dwelling adults age 55 and older who need nursing home-level care and are eligible for both Medicare and Medicaid. PACE organizations . . .
