10 Components Needed To Transition Hospitalized Adults With I/DD & Co-Occurring Mental Illness Ready For Discharge To The Community
A consumer-centered, cross-sector approach with 10 core components is more likely to result in a successful transition from a hospital to the community for adults with intellectual/developmental disabilities (I/DD) and a co-occurring mental illness. This population is at high risk of delayed hospital discharge, due to lack of appropriate post-discharge settings. Delayed discharges are associated with worse consumer outcomes and higher costs.
